Membership terms
The fine print
- The annual plan fee is non-refundable and non-transferrable.
- The payments for treatment rendered, and the plan cannot be combined with dental insurance or any other discounts.
- Discounts cannot be applied to treatment by a specialist or any other office.
- Membership auto-renews each year; a $50 re-enrollment fee applies if it lapses.
- Monthly payment option available for an additional $5/month per patient.
- Excludes sleep apnea, implants, cosmetic, and full-mouth cases.
